APPOINMENTS AND SCHEDULING
If you need an appointment, you may call our main office, and we will be glad to assist you in scheduling your appointment. Please have your information, including all employer and insurance information available when scheduling.

Unless it is of a dire need, Dr. Arnold is unable to see any patient without an appointment. The schedule is filled well in advance, and unless you have an injury that is recent, please call ahead for an appointment time. Also, please do not bring anyone with you to be seen, unless they too have made an appointment for that time. We will do our best to work within your needs.

We request that all appointments be cancelled no later than 24 hours in advance as a courtesy to Dr. Arnold and the rest of our patients. We understand that emergencies may arise from time to time, however, if adequate notice is not given, you may be responsible for payment of any non-cancelled appointments.

Please be aware that all appointments are scheduled with an expectation of time involved. However, we are not always able to anticipate all of our patients needs. There are times where a wait time may be involved. We understand you may be anxious, however, please remember we will spend as much with you as with the remainder of our patients. We will make every effort to inform you of any potential delays in your appointment time.

CLINIC FORMS
Downloadable Forms
Prior to your first visit, you should download these forms, fill them out completely and bring them with you:
Patient Information Form

Patient History Form

Note: The forms above are viewed through Acrobat Reader.

Please bring the following items to your appointment with you:
  1. All insurance cards that you may have. If your insurance requires you to have a referral from your primary care physician, you should have this before your visit.
  2. Any studies, such as X-Ray's or MRI's that you may have previously had
  3. If another orthopaedic physician has seen you before, or if you have ever previously had orthopaedic surgery, you should obtain your clinic notes from that physician and your operative reports and bring them with you.
  4. If your injury stems from an injury at work then you should bring all information pertaining to your claim such as: Claim Number, Claim Adjuster, Employers Name, Address and Phone Number, the date of the injury and the exact circumstances surrounding the injury, and any hospitals or other physicians who treated you for the injury
  5. If your injury stems from a motor vehicle accident, you should bring a copy of the liability insurance involved.
  6. If your injury stems from an accident at school, you should have your coach fill out the accident form at the first available opportunity and submit it to our office.
  7. Please bring a detailed list with accompanying dosages of ALL of your medications you are currently taking
BILLING INFORMATION
Payment in full is expected at the time of your visit. Exceptions include workers compensation, Medicare, and preferred insurance. You may contact our office for a complete list of all insurance plans that we accept. Since most insurance companies do not reimburse the total amount due, you will be responsible for any charges that are not covered under your plan. For your convenience, we accept Visa, MasterCard, American Express, cash, or check.

If your injury is a result of a motor vehicle accident or if it is currently involving litigation, you will still be responsible for you bill at the time of service with your private insurance. We do not accept liability claims as compensation.

If your question involves a service that Dr. Arnold referred you to such as a local surgery center, or therapy center, these parties are not associated with AOSM. You should direct all inquires to the appropriate business offices

If you have questions involving your account, please contact our office and we will be glad to assist you.

PRESCRIPTION REFILLS
If you need a refill on your prescription, you may call our office Monday through Friday from 8:00 a.m. - 5:00 p.m. Please have the name and dose of the medication, the pharmacy with telephone number to call in to, as well as a number where you can be reached if any questions should arise. We make every effort to expedite all requests, but please be aware that we are also seeing and treating patients. All refill requests may take up to 48 hours to be filled. Check with your pharmacy to determine availability. Refills may not be made after business hours.

EMERGENICIES
If you have a problem after our normal business hours, then please call our office number and the operator will notify the physician on call with our group to contact you. Please be aware that the physician you speak with may not be familiar with your circumstances, therefore you should be prepared to give some background information to your problem.
